At the time of booking your appointment, please let us know if you are pregnant or believe you may be pregnant.

Allergies/ Disorders/ Medication

Before your appointment, please let us know of any allergies or disorders that may affect your examination as well as any medication that may be relevant (such as as asthma, blood thinning, diabetes, etc medication).

Preparing and Arriving for your Appointment

At the time of booking your appointment, our staff will advise you if there is any specific preparation required for your appointment. Please adhere to any advice provided.

We recommend that you arrive at least 10 minutes prior to your scheduled appointment to ensure that your paperwork is processed.

What to Bring to your Appointment

• Referral letter/ Request form from your doctor or health care provider
• Medicare Card
• Any concession cards you may hold (such as Health Care Card, Pensioner Concession Card)
• Any previous medical imaging/radiology scans that may be relevant to your appointment
• If relevant, TAC/ WorkCover claim number
